杭州疾控中心防艾干预 到娱乐场所劝人用安全套

据都市快报 昨天（22日），由杭州市疾控中心副主任陈树昶领衔的《杭州市娱乐场所服务小姐艾滋病相关行为干预研究》课题，通过了由杭州市卫生局组织的省级专家组验收。
专家组对课题的评语是，“该项研究是杭州市首次针对娱乐场所工作人员，进行系统、科学的艾滋病相关行为干预。有助于总结出一套符合杭州市实际情况的娱乐场所综合干预形式，对有效遏止艾滋病经性接触途径传播，具有重要意义。”
这项工作由一支40人组成的疾控队伍完成。昨晚，本报记者独家专访了课题主持人陈树昶、上城区疾控中心传染病防治科胡锦峰科长、医生姚英。
抽查220位
娱乐场所工作人员
无一人感染艾滋病
“把课题放在上城区是有原因的，这是杭州最发达的区域之一。娱乐休闲业兴起比较早。”市疾控中心副主任陈树昶说。
这项研究2007年2月启动。在知情同意情况下，课题组先花了5个月时间调查了257名16-51岁的娱乐场所工作人员。
她们来自于上城区范围内的宾馆、酒店、夜总会、桑拿、休闲店、卡拉OK、歌舞厅、酒吧、洗脚屋、发廊、路边店等。调查结束，课题组采集了267份血样。当时检查出一些性病。
接下来的工作中，建立与高危险人群的联系，登记了娱乐场所工作人员的联系方式。每周通过向目标人群发送手机短信，进行性病、艾滋病知识宣传。
市、区疾控中心专业人员以街道为单位，每半个月轮流深入到场所提供服务一次，主要是提供上门的免费检测和咨询，检测项目包括HIV抗体、梅毒抗体、乙肝表面抗原和丙肝抗体。
检查出患病者，将送到杭州市第三人民医院成立的规范化性病门诊，专业治疗。
2008年2月，课题组开始终末调查，共对262名娱乐场所工作人员问卷调查，采集血样220份。220位娱乐场所工作人员中，无一人感染艾滋病。
陈树昶说，这说明杭州相对于我国其他城市，目前娱乐场所工作人员的艾滋病感染率是很低的。但是，我们也检出多例梅毒等性病感染者，防控工作还是不能放松。
花了一年时间
深入娱乐场所
课题组主要在上城区348家休闲娱乐场所开展干预工作，基本上每周去两三次。2009年后，频率为每周一次。
“现在的工作比较顺了，干预人群突破了2000人。最初开
始干预的时候，实在是太难了。经常被赶出门。”上城区疾控中心传染病防治科胡锦峰科长说。
这项课题，最早从2006年5月份开始的，花了一年时间才深入娱乐场所。刚开始，我们夏天一个下午跑20多家店，家家吃闭门羹。被骂神经病，被说是免费检查然后拉去看病的医托。更多的，她们是担心我们举报到社区或上级部门去。扫街时常被放鸽子，店主告诉我们第二天来，结果去的时候店面故意关门。娱乐场所人员流动性大，有时候建立的联系也会断掉。
另外，因为大部分是晚上调查，有些娱乐场所的保安很不客气。我一个男同志，往往要陪2位女同志一起上门做工作。
我们刚开始通过专门培训，学习怎么和她们打交道。比如握手，尊重她们，帮助她们。后来，花了一年时间，通过社区、协警、计生人员等共同努力，现在情况好多了。

与高危人群打交道
“主动上门联系的，从最近几个月开始有了。”上城区疾控中心姚英医生说了一个例子——
湖南女子，30出头，前天主动带了小姐妹找到我抽血化验。女子说，她来杭州五年了，一直在街边休闲店打工。男友的工作不固定。以前有病也是扛着，去年参加我们干预课题后，才把病慢慢治好。
她说，自己过两年就和男友回老家开个小店，重新生活。
姚英医生，参加项目一年，负责上城区的小营、望江两个街道的休闲娱乐场所，已建立联系的对象369人。
每周，姚英都会抽出一天时间“扫街”，沿街一家一家休闲店、按摩店等访问过去，进去聊天，认识一些娱乐场所工作人员，聊上天，然后再宣传艾滋病防治知识。帮她们检查化验，后续帮助治病。
有时候，也有主动联系集体抽血的。
12月10日，姚英的工作目标是健康干预小营街道上的一家大型桑拿中心，以下是她的描述——
下午三点，我约好负责抽血的蔡医生、公共卫生助理员王大姐，一起到目的地。事先，桑拿中心的经理打电话给我的，说最近店里刚招聘了38位新员工。
我们要配合她们的工作时间，晚上她们比较忙，白天在睡觉。傍晚的时间做干预工作，最合适。
这家桑拿店我打交道半年了，建立了信任。我们一进去，经理就把我们安排在一个小房间里，组织员工5个一组进来调查。我们有分工，我给她们填写调查问卷，小蔡抽血，王大姐解答一些问题。
进来的姑娘都蛮年轻的，长得蛮好。最小的1992年出生的，大部分是80后。她们有的刚来上班，还穿着自己的衣服，蛮时尚、漂亮的。
刚进来时，气氛有点僵，不一会儿大家就嘻嘻哈哈了。我们事先培训过心理学，先和她们搭讪，比如说，“外地来杭州打拼，真不容易啊”。拉近与她们的距离。
问卷都是一些私密问题，比如个人情况、对艾滋病的了解程度、性生活情况和对安全套的使用程度等问题。
一个女孩说什么也不愿意写个人情况，她生气地说，你们要把我报告上去的！
还有女孩不愿意接受我们赠送的避孕套，她说是做正经工作的，不需要这些东西。

花了大概4个小时，38人的基础调查和血液采样完成了。

男性工作者艾滋病防制
课题主持人陈树昶说，经过课题组三年的干预，从以下一些数据中能看出一些成效：
艾滋病相关知识率从干预前的13.23%提高到33.97%；
使用安全套情况，从干预前的70.50%提高到88.9%；
最近一个月与配偶/同居男友每次发生性行为时均用安全套的比例从干预前的6.42%提高到干预后的46.67%；
从不使用安全套的比例从29.79%下降到15.56%；
选择自己买药治疗的比例从干预前的79.35%下降到干预后的23.71%。
陈树昶的下一个课题是研究《男性娱乐服务行业艾滋病相关行为干预研究》，“从最近几个月的调查看，杭州男性娱乐服务行业的艾滋病发病率，大大高于女性娱乐服务行业。今年抽查出19对男性艾滋病患者。”陈树昶说。 （作者 张姝 施水泉）
