慢性艾滋病早期中医药治疗保障生命论证

 
    在世界艾滋病日前夕，记者在湘雅二医院感染科艾滋病研究室了解到，该研究室郑煜煌教授等专家参加 “艾滋病并发结核分支杆菌感染的流行病学和治疗策略研究”、“艾滋病并发丙型肝炎的治疗研究”和“中医药防治艾滋病综合研究”等三项“十一五”重大专项研究，将于下月结束。经过近三年的努力，该三项课题均已取得了重要的成果。 
  
探索艾滋病并发结核治疗途径 
    机会感染是艾滋病患者发病和死亡的主要原因，结核病是我国艾滋病患者最为常见的机会感染之一。研究显示艾滋病患者中肺结核的发生率在16%～31％之间，全球3400万HIV/AIDS患者中约有1250万人合并结核菌感染。有超过一半的艾滋病病人最终因结核病而死亡。因此，预防和控制艾滋病病毒和结核病的双重感染成为我国极其紧迫的公共卫生问题。 
    湘雅二医院和各合作单位对来自我国各地的1139例艾滋病人，用痰菌检验、X光胸片检查、结核抗体或病理检查，发现其中合并结核病者287例，合并感染率为25.20%；对1023例结核病人进行HIV抗体检测，发现合并艾滋病者16例，合并率为1.58%。发现静脉吸毒传播的艾滋病人更容易合并结核病，有吸烟史或其它慢性肺部疾病史的结核患者中得艾滋病的比无该类病史的患者更多。 
    我们对发现的部分艾滋病合并结核患者分别于2009年4月-2010年4月先后启动规范的抗结核治疗和抗艾滋病病毒治疗,随机分组采用不同的抗病毒启动时间和不同的治疗方案，并对每位病人定期进行各项常规化验、生化检查、痰菌检验和X光胸片复查，CD4细胞和HIV病毒载量复查，并严密观察病情的变化。至2010年11月，除2人因去外省打工停止用药外，所有接受治疗的病人均已按要求完成了抗结核治疗和定期追踪，病情明显缓解；CD4细胞上升、HIV病毒载量明显下降，痰菌检验持续阴转；少数人用药有副作用但均已纠正，无一人恶化或死亡。目前仍在坚持抗艾滋病病毒治疗中。 
    本课题的顺利实施和圆满完成，是我国首次由多个临床科研单位联合开展对艾滋病最主要的机会感染——结核病的防治研究，初步揭示了我国艾滋病人群中结核病的流行状况、影响因素和发病特征；探索了适合我国国情的艾滋病合并结核病人群的抗病毒治疗和抗结核治疗的最佳策略；为降低我国艾滋病合并结核感染的发病率和病死率，以及控制艾滋病和控制结核病的疫情发展起到重要的推动作用。 
  
着力降低艾滋病合并丙肝炎者病死率 
    由于人类免疫缺陷病毒（HIV）和丙型肝炎病毒（HCV）都可以通过血液传播、性传播和母婴传播，两者合并感染相当常见，尤其在静脉吸毒感染艾滋病者中更高。近年来我国HIV合并HCV感染者的发病率和死亡率呈现上升趋势。这两种病毒的相互作用，不仅加速了艾滋病与丙型肝炎的病情进展，还直接影响抗病毒治疗的效果。但是当前对于两种病毒合并感染者的抗病毒治疗方案和时机尚无统一意见，这些问题已成为当前研究的热点和难点。 
    湘雅二医院和各合作单位对来自我国各地的1251例艾滋病人，以统一的流行病学调查表进行登记；并进行丙肝病毒抗体检测、HIV和HCV病毒载量检测、CD4细胞检测、各项常规化验和生化检测；用SPSS13.0电脑软件对不同因素下HIV合并HCV感染的情况进行统计学分析。结果发现艾滋病合并丙肝病毒感染者398例，合并感染率为31.8%。其中通过静脉吸毒感染者的合并感染率为81.0%(321/398)；30～45岁年龄组的中青年艾滋病人和无正当职业者中合并感染率也明显高于其他病人。 
    对其中一部分艾滋病合并丙型肝炎患者分别于2009年7月-2010年8月先后启动规范的抗丙型肝炎病毒治疗和抗艾滋病病毒治疗,以罗氏公司进口的派罗欣干扰素和利巴伟林治疗丙肝，随机分组采用不同的抗艾滋病毒启动时间和不同的治疗方案，并对每位病人定期进行各项常规化验、生化检查、CD4细胞计数、HIV和HCV病毒载量复查，并严密观察病情的变化。至2010年11月，除3人停止用药外，所有接受治疗的病人均按要求进行了规范治疗和定期追踪，病情明显缓解，两种病毒载量均降到可检测水平以下；少数人用药有副作用但均已纠正，无一人恶化或死亡，目前仍在继续坚持治疗中。 
    本课题的顺利实施和圆满完成，是我国首次由多个临床科研单位联合开展对艾滋病合并丙型肝炎的防治研究，建立了全国HIV合并HCV感染研究队列；结合我国免费抗病毒治疗策略，探索了适合我国HIV合并HCV感染人群的抗病毒治疗方案和最佳治疗时机；观察了不同抗病毒治疗方案对HIV合并HCV感染者肝脏损害情况及相关对策，对降低我国HIV合并HCV感染者的病死率和提高HIV与HCV双控制效果有重要的科学价值和临床意义。 
  
中医药防治艾滋病取得新进展 
    当前推广的抗艾滋病病毒治疗能有效地抑制HIV的复制，延缓病情和减少并发症，但是部分接受治疗者可能出现病毒耐药，并不得不忍受药物的毒副反应。另外一些暂未抗病毒的HIV感染者常有一些临床症状如低热、乏力、不适、纳差等需要对症处理。在上述感染者中使用中医药治疗具有其独特的优势，某些中药提取物及其组分具有增强免疫功能和抑制病毒的作用，并能克服艾滋病的不适症状和抗病毒治疗药物带来的副作用。同时我国广大患者对中医中药较能够接受，依从性好；且中药资源丰富，价低效好，毒副作用小，可长期服用, 没有耐药性。 
    湘雅二医院和各合作单位自2009年6月至2010年11月，对来自全国各地的近2000名艾滋病人进行了病情观察和相关检查，对其临床表现进行中医辨证论治，并选择了其中一部分合适的病人，开展了规范的中药治疗。期间使用几种比较成熟的中成药或组方对抗艾滋病毒治疗中常见的副作用如恶心呕吐、腹部不适、血相降低、血脂升高等，并使用几种中成药或组方对抗艾滋病患者常见的并发症。并对每位病人定期进行各项常规化验、生化检查、HIV病毒载量和CD4细胞复查，并严密观察病情的变化。至2010年11月，所有接受治疗的病人均按要求进行了规范治疗和定期追踪，病情明显缓解，各项实验室指标有所改善，无一人中途停止服药、无一人病情恶化或死亡。 
    本课题的顺利实施和圆满完成，是我国多个临床科研单位联合开展对中医药治疗艾滋病的大规模研究，摸索出了几种较好的治疗艾滋病的中药组方和治疗方案，探索了适合中国国情的中西结合治疗艾滋病的有效途径，并研究了中医药治疗艾滋病可能的作用机制。为建立中医药或中西结合治疗艾滋病常见症状和降低艾滋病发病率和病死率的可行方案，为祖国医学治疗艾滋病探索了一条新路子。

　　昨日是第23个世界艾滋病日，这一天全世界各地区都举行了各种各样的防艾宣传活动。昆明市卫生局副局长、防艾局局长李华生昨日透露，目前住址在昆明市的HIV感染者、病人已经超过1万人，报告数位居全省之首，疫情还有进一步上升的趋势，而最易感染艾滋病的人群是缺乏防艾知识的外来务工人员。

　　缺乏防艾知识

　　昨日上午，省总工会、昆明市总工会和昆明市防艾委等单位，联合在东风广场举行了世界艾滋病日宣传活动。昆明市部分非公企业应邀组织了本单位的进城务工人员到现场参加活动。今年17岁的赵海森来自石林县西街口乡，在昆明市某家政公司打工已经有七个月的时间。和他来自同一个村子又在一起工作的赵福明比他大一岁，两人都是初中毕业后出来找工作才来到昆明的。问到对艾滋病有多少了解，两人均表示以前听说过，但是不太清楚究竟是一种什么病，也不知道艾滋病的传播途径。

　　赵福明说，几天前他们才接到参加活动的通知，昨天该家政公司共有20多位农民工参加了宣传活动。一位来自石林县的彝族年轻女子，听到记者询问有关艾滋病的情况时，红着脸笑而不答。在活动现场，记者随机采访了十位农民工，他们大部分都说不清楚艾滋病的传播途径，对于艾滋病的了解，也仅仅停留在“这个病医不好”的层次上。这种情况和卫生部门调查了解到的信息基本相似，很多进城务工人员、长途卡车司机因为不知道采取防护措施，在无保护状态下的性交易行为导致他们感染了艾滋病病毒，成为艾滋病病毒携带者、病人。并且，他们很有可能在无意识的情况下，又将艾滋病病毒传染给自己的伴侣或其他人。

　　省传染病专科医院感染大科主任汪习成指出，在艾滋病从高危人群向普通人群蔓延、性传播成主要途径的当前，缺乏艾滋病预防知识，又恰好处在性活跃期的广大进城务工人员，很有可能成为艾滋病的感染者和传播者。因此，向农民工群体普及预防艾滋病的相关知识，非常重要也非常紧迫。

　　住址在昆的过万

　　染病人数居全省之首

　　昆明市防艾局局长李华生透露，2009年全球新增艾滋病感染者260万人，180万人死于艾滋病。截至2009年年底，全球共有艾滋病感染者3330万人。这一数据虽然庞大，但也充分体现了近三十年来，人类在抗击艾滋病过程中取得的重要成绩。“研究人员预测，如果不进行预防，到今年全球将有7500万人受到感染。到2020年，全球将有1亿人死于艾滋病。”

　　李华生介绍，截至今年10月底，中国累计报告艾滋病病毒感染者和病人37万余例，其中病人13万余例，死亡6.8万余例。我省是全国报告艾滋病病毒感染者、病人最多的省份。从1989年到2009年累计报告艾滋病病毒感染者、病人72939例，艾滋病病人14954例，死亡9291例。去年，昆明市艾滋病病毒感染者、病人报告数位居全省16个州市之首。

　　“自从1987年在美国旅游者中发现首例艾滋病病人以来，到今年10月底，昆明市检测发现的感染者、病人总数已达1.5万多例，现住址为昆明市的感染者、病人总数超过1万例。”他说，昆明市所辖的14个县（市）区、3个开发度假区均已发现艾滋病病毒感染者。从传播途径来看，昆明市血液传播、性传播、母婴传播三种途径并存，疫情呈现上升趋势，流行地区差异较大。

　　和全国总趋势一样，最近几年，昆明市性传播也逐渐成为主要途径，男男同性性传播所占比例快速增加，艾滋病流行的危险因素广泛存在，艾滋病在普通人群中进一步扩散。为了进一步遏制艾滋病疫情的快速上升，昆明市已经建立了30个艾滋病初筛实验室、202个快速检测点，8个国家监测哨点和11个省级监测哨点，形成了覆盖全市各个县（市）区的艾滋病监测检测网络。

　　昆明市总工会主席戚永宏在活动上表示，举办宣传活动的一个重要目的，是让广大职工群众知道，艾滋病在全球范围内是能够加以控制和预防的；预防艾滋病很重要的一条，就是每个人都要对自己的行为负责。活动上，一位企业职工代表宣读了倡议书，倡议全市职工和广大农民工主动参与，增强预防艾滋病的意识；洁身自爱，培养健康的行为生活方式；互助互爱，共筑抵御艾滋病的坚固长城。生活新报 记者 刘启银

　　昨日，盘龙区政府在桃源广场举行了以“反歧视·人文关怀”为主题的防艾宣传活动，向过往市民发放防艾、禁毒等宣传资料以及计生用品等。

　　盘龙区防艾办公室相关负责人介绍，以“反歧视·人文关怀”的主题的防艾宣传活动，已经持续近三年，除开在广场向市民宣传防艾、禁毒等常规“动作”外，每年还会根据实际情况开展相应活动。

　　在昨日桃源广场主会场，盘龙区以防艾办公室牵头，包括卫生、禁毒、青年总工会等在内的17个部门在广场纷纷设点，向市民发放印有防艾字样的环保袋宣传防艾知识。据了解，除开主会场，盘龙区其余37个成员单位还在各自的辖区范围内举行宣传活动；12个社区的工作人员还将入户宣传，并到艾滋病患者家中进行慰问。鉴于目前盘龙区辖区内施工工地众多，盘龙区筑建局相关领导还组织疾控站专业人员到工地上为工人讲授如何预防艾滋病的知识。 生活新报 记者 姚宇

　　昨日下午，在云南省滇剧院内，一场专门为城市流浪人员举办的演出，不时引来阵阵欢笑声。在世界艾滋病日当天，由云南省家馨社区儿童救助服务中心发起的“预防艾滋 从我做起”文艺演出活动，请来了昆明市近百名街头流浪人员当观众，活动主办方旨在通过这些文艺节目，告诉他们什么是艾滋病，如何预防艾滋病。

　　来自家馨儿童中心的孩子和云南大学、云南民族大学等多所高校社团的学生，在近两个小时内为大家献上了十几个节目，其间还穿插进行艾滋病病毒感染者现身说法、艾滋病预防知识有奖问答等活动。

　　家馨儿童中心主任陆超群表示，这是他们第一次尝试将流浪人员请进剧院观看演出，并通过文艺演出的方式，提高他们了解艾滋病的兴趣和意识，让他们掌握预防艾滋病的相关知识和技巧。

　　“流浪人员在街头的生活充满了艰辛和挑战，他们中的一些女性和儿童，还可能有遭遇性侵犯的经历。”他表示，作为一家专门从事城市流浪儿童综合救助服务的非政府组织（NGO），虽说不能改变他们被迫流浪街头的困境，至少希望他们能学到一些自我保护的技巧。 生活新报 记者 刘启银

　　前日，40多位河南的艾滋病患者、感染者来到北京，表达改善自身生活现状的诉求。一位患者家属说，他们愿意面对媒体，就是希望在艾滋病日这天，得到更多关注，“如果这一天不能让更多的人了解到我们的处境，其他的日子就更难了。”
