抗逆转录病毒药可大幅降低艾滋病病毒传染率。


  一项由多国科学家参与的研究发现,艾滋病病毒(HIV)感染者如果在患病初期就接受抗逆转录病毒(ARV)治疗,则可以将病毒传染给性伴侣的几率减少96%。美国国家过敏与传染病研究所(NIAID)所长Anthony Fauci表示,这一研究结果有着重大的政策含义,同时“再一次凸显了(艾滋病)早期治疗的多重收获”。

  NIAID斥资7300万美元资助了这项为期6年的研究工作。该研究所曾打算再为其提供4年的经费,但是由一个独立监察小组进行的中期分析使得NIAID决定提前结束研究并宣布相关研究成果。

  由HIV预防试验网络(HPTN)在9个国家的13处地点开展的这项研究,共招募了1763对伴侣(其中97%为异性恋),每对伴侣中有一人刚刚感染了HIV。他们当中没有一个HIV测试呈阳性的患者曾接受过ARV治疗,并且其血液中的CD4细胞数为每毫升350至550(正常值应大于600)。所谓CD4细胞就是表面有CD4分子的T淋巴细胞,其含量是衡量人体免疫系统健康水平的重要指标。

  在这项研究中,其中一半的受试者立即接受了治疗,而另一半受试者则等到CD4细胞数下降至250,或发展出与艾滋病(AIDS)相关的症状才开始接受ARV治疗。并且试验中的每个人都收到了免费安全套,安全性行为辅导,以及针对其他性传播疾病的治疗。

  中期分析显示,共有39人被HIV感染,而遗传测试显示其中有28例感染与他们的伴侣有关。而在这28人之中,只有一个人的伴侣被分在了立即治疗小组。

  较早前的观察性研究已经显示了ARV治疗的类似影响,即减少异性恋伴侣之间因性传播HIV的可能性,然而新的研究则首次在一项随机对照临床试验中证实了药物的预防作用。主持这项所谓HPTN 052研究的教堂山市北卡罗来纳大学的Mike Cohen表示:“这仅仅适用于异性伴侣。”Cohen指出,他们曾希望将男性与男性发生的性行为也包括在内,但因参与的志愿者太少而作罢。

  当前由世界卫生组织发布的艾滋病治疗指导方针要求在每个人的CD4细胞数量降至350或更低时提供治疗。发达国家的大部分指导方针则建议在350至500之间便开始治疗,然而实际上,那些有医疗保险或有钱的人在任何CD4细胞数量下都可以开始治疗。

  目前,研究参与者已被告知研究结果,研究人员计划对两组参与者继续跟踪观察至少一年。

Interim analysis, a total of 39 people were HIV infected, but genetic testing revealed that one in 28 cases related to infection and their partners. In these 28 people, only one partner was in the immediate treatment group.

Earlier observational studies have shown similar effects ARV treatment, a reduction of between heterosexual couples because of the possibility of sexually transmitted HIV, but the new study is the first time in a randomized controlled clinical trials the drug proved prevention.
