李小萌：今天已经是第21个世界艾滋病日了，这也正是我们今天节目的主题，先来看一组国际国内相关新闻。
【今日国际国内新闻】
解说：今天是世界艾滋病日，今年的主题是遏制艾滋，履行承诺。卫生部昨天公布，截至今年9月30号，我国累计报告艾滋病病例26万多例，性传播已经成为主要的传播途径。
据中国艾滋病防治联合评估报告估计，截至2007年底，我国现存艾滋病病毒感染者和病人约70万，其中艾滋病病人8.5万。而卫生部日前工作的统计数据显示，截至今年9月30号，我们累计报告艾滋病病毒感染者和病人264302亿。其中艾滋病病人77753例，报告死亡34864亿。
为了迎接世界艾滋病日的到来，在印度西孟家拉瓦首府加尔各答11月30号举行了一场名为红丝带马拉松的长跑活动，旨在提高人们对艾滋病感染者成低龄化趋势的重视。
据统计，全球3320万感染者当中，15岁至24岁年龄段的就有一千万，15岁以下的有250万。同一天，在西班牙首都马德里，人们将印满人相的巨副红丝带悬挂在市中心的阿尔卡拉板上，以此为世界艾滋病做宣传。
今年世界卫生组织多次发表报告指出，全球艾滋病防治出现了重大变化，新增艾滋病病毒感染人数和死亡人数都有所下降，在疫情严重的非洲地区，艾滋病防治有了突出的表现。在南非政府采取各种措施，促使艾滋病治疗药物的价格大幅下降，并公布贫困地区的艾滋病患者接受治疗。
在乌干达，一场名为乌干达关怀的行动日前也在全国范围内展开，对人们免费检查是否感染爱滋病毒。不过世卫组织也提醒说，全球艾滋病疫情蔓延的趋势尚未出现，防治艾滋病行动仍是一项长期而又艰巨的任务。
【演播室访谈】
李小萌：1981年12月1号在美国确诊了全球第一个艾滋病病例，到1988年12月1号成为第一个世界艾滋病日，在这27年当中，艾滋病跟人类如影随形，也夺走了上千万人的生命。但同样在这27年当中，人类跟艾滋病抗争的步伐没有停止过。
张羽：我们不能否认的是，到今天为止，艾滋病仍然是一个绝症，我们没有办法去解决它。在接下来的时间里，我们将通过一些历史的影像来回顾一下中国面对艾滋病的一个进程。我们会通过对一些大家陌生或者熟悉的面孔背后的故事的探究和展示，来探究在中国面对艾滋病变化的进程当中，也许与我们每个人不无关联的一些变化。
李小萌：我们下面看的这组影像应该说是来自于1997年，那也是我们可以找到的比较早的记录。当时针对艾滋病的医疗环境，以及当时的患者面对什么样的境遇，一起来看一下。

徐莲芝：昨天晚上怎么样老张？怎么样，老张？
老张：这两天我也是感觉有一点好转，但是就是这个星期，在下午三点老是要恶心。
徐莲芝：我看一看，我想这样，明天早上我请儿研所的同志来，他会给你介绍这个药，这也是偶然的发现，本来是在小儿腹泻的。是因为我那天开会，他腹泻我没办法，我就跟人讲，我说你有什么好办法帮帮我，他跟我讲，他说那你试试这个，但是这是一个新的，意外的发现，我前一个病例就非常遗憾。
老张：染上了这种鬼病，气死掉。治这个病没用，现在后悔莫及了。
徐莲芝：我可以问一下一个问题，当时您是怎么得的这个病。
老张：我当时几个老板一起去的家属都在家里头，到泰国那个性开放的世界，肯定要到外面去玩，也不知道什么时候，得了这种病。
徐莲芝：喂，你好，对，你有问题讲吧。我跟你讲，尽管你皮肤破了一点，或者真的有什么倒刺，这样的小口，那你想，那个艾滋病毒怎么就那么巧，它就一定要到那里面去呢，不是说你说外国人多，来个老外我就都怀疑他，他的衣服我也不敢碰，他的手也不敢握，我看没有必要，不到这种程度，好吗，那就这样，再见。
徐莲芝：老张把这药吃了吧，这是下午的药，
老张：太多了，这么多大黄是什么东西，多是没事，钱多了没事，药多了没事，就是病不好才糟糕。
徐莲芝：着急，是吧？
老张：着急。有点着急了。
徐莲芝：今天15号。不要着急老张，终是得慢慢地来。
老张：太慢。
患者：肯定是没干好事，肯定是在外头乱搞了，干好事能得这病吗？当这个现实降临以后，接受不了，怎么接受啊，不可能是我，会不会搞错啊。几个月了，那时候两三个月了，反复要求再查，实际上大家也在反复查，但确确实实是接受不了，以前没有病的时候，也想办法摆脱过这种生活，人不人鬼不鬼的生活方式，那时候的那种生活，就想到了有一天肯定会得艾滋病，只是一种想象，但是又一想，这种病肯定离我还遥远，其实一点也不远，这不就感染上了。人这一辈子就那么回事，生生死死，怕死也得死，就是死的方式不同，我这种死法太悲哀了。
徐莲芝：就把这当家吧。
患者：不是当家了。也没办法。
徐莲芝：要么哭，要么发泄，人有的时候就这样，不管怎么样都要发泄一下，因为苦啊，压抑啊。

李小萌：看当时的这些影像肯定有很多感慨。因为那个时候人们刚刚开始了解艾滋病，模模糊糊的有些概念，比方说开始的时候，艾滋病的这个艾字，不是现在的这个草字头，而是这个爱情的爱，就是说把它解释为，凡是艾滋病，都是和不洁的性爱有关的，但是现在多数人都知道它有三个传播途径，血液传播、性爱传播、母婴传播，有很多人是无辜和被动的。
张羽：确实，我们看到11年的光影，虽然记录得很平实，但是完全可以感受到，当时的病人和医生对艾滋病那种陌生、恐惧和挣扎，确实其实1997年是中国艾滋病防治一个非常关键的点，因为从那年开始艾滋病在中国呈现出一种蔓延的势头。我们看到的不仅是影片当中的这种病人和医生这种陌生和恐惧，实际反映出的是在这个事件完全到来的时候，我们整个社会准备不足，甚至有些惊慌失措。
李小萌：是，在那个时候，刚刚人们知道有艾滋病，接触它。即便是现在非常有名的抗艾专家，接触病人的时候，都要全副武装起来，戴上手套，回家之前把全部衣服脱掉，要洗个澡。到了现在，经过20多年过去以后，在艾滋病第一村，那的医生已经知道，当跟病人常规接触的时候，不用什么太严重的防护措施，只有进行治疗了才需要戴上手套，这也是这些年不断地进行宣传，发生了巨大的一个改变。
张羽：而且我想细心的观众一定会发现，收留艾滋病感染者的治疗点条件非常简陋，确实经过这么多年，我们来看，刚才小萌讲到是，大家的意识软件，我们现在也看到这些年硬件的变化，首先从医护人员，我们从当初1998年的5000多，现在到了23000，全国合格的检查实验室达到六千多个，确切的时间是170多个，近80%的县以上的机构具有了艾滋病病毒的检测能力，检测点已经达到了4300多个，全国推广安全套覆盖项目已经增加到1900多个，门诊达到了五百多个。这是十年来的进步和变化，但是应该说面对现在艾滋病发展的势头，显然这些硬件和覆盖的程度还是远远不够的。
李小萌：而硬件这种是一个方面，其实社会的软环境对艾滋病人什么样的态度，什么样的包容度也更重要，有人说一个人如果得了艾滋病是不幸，如果不能被社会接纳和得到应有的救治，就是一个悲剧。咱们来看看在2000年的时候有一个短片，记录了那个时候即便是给疑似为爱滋病患者都会遭遇到什么样的境遇。

解说：余梅芳，浙江省宁海市张家镇人，2000年2月，她因患椎间盘突出，在山西省忻州市地区医院做手术。
余梅芳：到2号早上，大概有十几点了，主治大夫过来，他说今天给你换个病房，他说你在这个地方休息不好，从2号窗户上面趴着好多人看，这个门就关不住，人就冲进来，一看，就是她，就是她。到4号中午，我的主刀大夫翟主任来了，我说你坐吧，他也不敢坐。他说我们医院给你看的是，说得好听一点，你50%得了艾滋病了。我当时就想一下子爬起来，我想拉住他，我说我不可能的，他赶紧往后退，他说你一定要相信科学，我倒是希望你别得艾滋病，但是科学是明摆着，你就是得了艾滋病，他说你看着办吧，你要是不出院，从今天开始也不输液了，也没人来管你了。
解说：从那天开始，医务人员就再也没有来过病房。心有余力的余梅芳先后来到忻州地区性病与艾滋病检测中心，山西省艾滋病抗体检测确认实验室做了三次HIV抗体筛查，结果都是阴性。
记者：您是怎么知道忻州市出了个怀疑有艾滋病的这个事的？
群众：我也是听他们传过的。
记者：听谁传过的？
群众：是这个社会上小道消息传过来的。
记者：余梅芳认识吧？
群众：最初我也认不得她，只知道是来买瓜子的，后来人们都指指划划告诉我就是她，得了艾滋病了。
记者：你知道传说她得了这个病以后，你还愿意把瓜子卖给她吗？
群众：不愿意打这个交道，但也难免要接触。
记者：你卖给她，他给你钱你敢要吗。
群众：那一次不敢要。
记者：那次怎么回事？
群众：那次买了一块钱大豆。给我一块钱，我不敢要，我害怕。
余梅芳：同行是冤家，我这个生意卖的比较好，她卖不了，她就骂我，一直说我作风不好，骚货，这个意思你们懂吗，骚货，我站起来问她，我说你说什么，她说你没事人家怎么说你得了艾滋病，我一句没法说，我一句话没说，要是以前人家那么说我，我肯定跟她没完。
记者：你心里委屈吗？
余梅芳：心里委屈你有什么办法，我这个人很要强，你白天看我那个，我只有夜里躺在床上掉眼泪。躺在床上哭，我自己要是熬不下去，反正能活多久活多久，也就无所谓。不管到什么程度我无所谓，但是我还有两个儿子，所以说当时冒着瘫痪的危险我还是东奔西跑，必须要把自己洗清。
解说：就在手术后的两个月，余梅芳正式向山西省忻州市医院提出民事诉讼，诉忻州市地区医院和防疫站侵害其名誉权，索赔名誉及精神损失41万元。
【演播室访谈】
张羽：这是2000年发生的故事，其实可以说即使到今天，这样的故事还在重复的发生着。确实应该说，防治艾滋病最大的障碍并不是说我们的治疗手段，而是人们的恐惧与歧视。
李小萌：看这段影像的时候我们都在想，像她肉体上的痛苦，精神上的痛苦，希望到了今天，精神上的痛苦至少能够缓解，特别在翻阅历史资料的时候我看到，在新千年新春的时候，有一位艾滋病患者写给病友的一封信，他说我们是一个特殊的群体，我们得了一个特殊的病，但我们跟常人一样有爱，有自己对人生的梦想，他说我们发誓，不再让其他人感染艾滋病，让子弹在我的胸膛爆炸，不要再射向我其他的亲爱的同胞。
张羽：应该说对艾滋病这样一个事情，一方面现在我们还没有良药可以根治它，大家恐惧陌生。另一方面，大家不了解这个疾病的时候，容易把它蒙上道德审判的色彩，所以这反而使我们得了艾滋病的病人，或者感染者，他不敢公开去接受治疗。这样反而导致这样一个在群体在隐藏，而这种隐藏带来的威胁是非常巨大的。
李小萌：首先他们是受害者，让我们进一下广告，马上回来。

李小萌：欢迎回来，继续收看我们的节目，现在在这个世界范围内，防治艾滋病无非就是药物的治疗、疫苗，还有行为干预，而当药物不能够让这个病彻底被治愈，疫苗还在研制当中的时候，行为干预就显得越发重要，来看一个短片。

冉元林：当时县里面通知我们开会，开始去的时候，我们还不晓得怎么回事。
解说：这位警察叫冉元林。从事湖南澧县公安局治安大队的大队长，在这个会议上，澧县政府宣布，要在全县推广一个叫娱乐场所百分之百使用安全套的项目。
冉元林：我们感觉是不是打击卖淫嫖娼，是不是与我们平时的治安环境，是不是有些矛盾的地方。
解说：这个娱乐场所百分之百使用安全套项目，是卫生部和世界卫生组织合作开展的项目，目的就是为了防治艾滋病和性病的春播。
黄道海：社会有阻力也有压力。而且很多人有想法，卫生组织的防治艾滋病百分之百使用安全套，会不会认为，涉黄的事件多了，艾滋病人多了，会影响经济投资环境。
冉元林：一方面要求你使用安全套，一方面我要打击你，那绝对是矛盾的。这个矛盾怎样说呢，谁也说不清楚，我说不清楚。
解说：这是联合国使用的艾滋病流行的一般规律示意图，图中列举了不同的人群，这张图表明，预示艾滋病由高危人群向一般家庭妇女和儿童转移的最主要的途径。而这个娱乐场所100%使用安全套项目，就是为了堵住这条传播途径。在召开了一系列的动员会议以后，各个部门统一了意见，最终由治安大队大队长冉元林召开了全县各家娱乐场所的老板、经理、领班开始实施这个项目，会议的内容是动员他们配合这个工作。
冉元林：我觉得这个社会丑恶现象根治了，也就不存在性病，不存在艾滋病的传播，我觉得非常有道理，我们公安机关的打击是有限度的。在打击伏击这种现象同样存在的情况下，他们的防病是正确的，我是这样认为的，提倡防病是正确的。
解说：在娱乐场所，推广100%使用安全套，绝对不意味着政府不再打击卖淫嫖娼的违法行为，但在预防艾滋病的严峻形式面前，生命安全被放在了首位。
吴医生：那些小姐，第一次去都是冷冰冰的坐在那打牌，我跟她们讲话，她反正看也不看一眼，就这样，我们很受气的你不知道。然后就这样讲话，你找她们讲，那些小姐整个文化曾经比较低，她们就是完全不懂，跟她们讲，就是没听说过，艾滋病，什么啊，艾滋病没听说过，就这样，安全套没见过。
解说：这一天，吴医生去了一家娱乐场所，为从业人员体检。
吴医生：第一次发不要，谁都不要，不要，问你们干什么啊，我说我们不干什么的啊，不要，就是这样。但是只要有一个人开头，拿了两盒，然后其他的马上跑过来拿，我觉得他们实在是太少了，因为她们对这方面的知识了解太少了，那些小姐只要没接触过我们宣传的，根本不懂得安全套什么的，保护，不懂。
解说：根据最新统计，如果没有有效的防护措施。到2010年，我国的艾滋病感染人数可能达到1000万，防治艾滋病已经成为各级政府的重要工作。
【演播室访谈】
张羽：行为敢于是防止艾滋病的一个主要手段，我们可以借鉴其它国家的一些做法，比如说泰国，自从推广、发放安全套之后，艾滋病的概率就大幅度下降。但是我们可以看到，其实要进行深入广泛的战斗行为干预，首先是在于管理者，包括干预者理念的变化。如果说我们带着一些歧视的眼光，或者道德批判的色彩的眼光来看，很难进行真正的有效的干预行为。
李小萌：对，我们也可以从现在这个题板上来看，这些干预措施从刚刚开始实施到现在社会舆论的一种变化，一共有三种比较常用的干预措施。一个是在公共场所推广、发放安全套，就是百分之百安全套。另外是给注射吸毒人员提供美沙酮的维持治疗，另外是提供洁净的针具交换，这个就是给吸毒者提供洁净针具。像在公共场所发放安全套也好，好像是提供吸毒者的洁净针具也好，当时都有很大的争议，是不是在鼓励不洁性行为，是不是在鼓励吸毒，而到现在，社会的接触度在增加，人们也看到了政府在防治艾滋病方面采取的措施越来越务实了。
张羽：确实应该说政府行为干预这种观念的转变，包括这些实效的措施，可以看出我们政府面对艾滋病这样一个世界绝症的时候，以一种更加科学理性的态度去面对，应该说在这样一个过程当中，也有很多民间人士用超级的思维，真知灼见来推动了政府这样一个进程。

大玮：我叫大玮，我是一个同性恋，我也是一个艾滋病感染者，大夫把报告单拿给我的时候，他说你是呈HIV的阴性，当时眼泪就下来了。
解说：1981年，世界上发现的第一个艾滋病的人，是一个男性同性恋者。1989年，中国发现的本土第一例因性接触感染艾滋病的病人，也与多个同性有过性接触。目前，我国的男同性恋人群的艾滋感染率仅次于吸毒人群。
张北川：通过性行为被感染艾滋病的人，一共占中国所有的感染者接近30%。这其中就有三分之一是男性接触者。
解说：那么导致男同性恋群体容易感染艾滋的原因到底是什么呢，记者来到男同性恋聚集较多的酒吧，与同性恋者交谈，希望寻求答案，许多朋友告诉我们，同性恋群体，相对频繁更换伴侣的原因，是因为很难找到真正的爱人。
记者：这个群体当中，遇到真正的所爱的人比例高吗？为什么？
同性恋者：因为我们不可能像正常的人那样，我们认识新的同志机会真的很少。
张北川：我们在这个现实生活中，不断提到同性恋，而同性恋多了以后，他就不愿意产生感情，以后产生感情就是受伤害，谁愿意受伤害，于是就发生性关系，对于同性恋者来说，有巨大的好处，这个好处就是安全。
记者：安全？
张北川：对，你也不认识我，我也不认识你，两个人完了关系，大家互相都不认识。
记者：但这是不安全的？
张北川：但是他在那方面可以非常安全。
解说：而另一个让同性恋人群艾滋高感染的重要原因是无保护的性行为，由于男同性恋间的插入性行为，非常容易造成直肠黏膜的充血损伤，精液中的艾滋病毒可能通过破损黏膜进入血液循环或者淋巴系统。
记者：在你跟他有性接触的时候，你并不清楚这个人身体是否健康。
大玮：对。
记者：你是不是觉得如果你这个爱情为前提，就很难启齿去询问对方的生理情况，或者很难提出要求，做性爱前的措施，是这样吗？
大玮：我是有这样的想法。
解说：另外，中国传统文化对于性的敏感和羞涩，也使得无保护的性行为更容易发生。
记者：那时候知道安全套吗？
大玮：知道，这个知道，要是去哪个店里去买，我觉得可能会很不好意思。
崔子恩：在我知道的范围里面，有一些人还是懂得安全的性的知识的，还是懂得的，但是他操作起来不方便。他不能随身带着一个安全套，带上之后可能人家就会有歧视，会认为你在这个方面是一个放纵者。
记者：担心什么歧视呢？
崔子恩：担心发现的人，因为中国人差不多每个人都是检察官，都会知道，你柴静的包里带个什么，什么口红，大家都用透视的眼睛。
记者：安全套在大多数的眼睛中意味着什么呢？
崔子恩：意味着性，而不是安全。
解说：七年过去了，张北川一如既往，在他的努力下，越来越多的同行者加入了进来。周生建，重庆榆中区计生办主任，他是最早介入同性恋群体艾滋专业的官方人员之一。
周生键：绝大多数同性爱都已经结婚，或者准备结婚，平均一个同性恋或和双性恋，和五个异性恋男性有性接触，他们的疾病在他们当中蔓延的疾病也可以通过异性和他们的妻子向这种普通人群蔓延开来。
【演播室访谈】
张羽：尤其是男同性恋人群，是艾滋病的一个高危人群。现在对于男男同性这样一个性行为的人群的规模，包括流行的趋势和他们发生特点的调查，现在还缺乏很多的数字、依据。其实这需要我们在艾滋病的干预过程当中，给大家一个更加宽容，更加关爱的环境，这样才是我们采取任何干预性的基础。
李小萌：对性的取向，我们不加评判，但是在有安全防护措施的情况下，进行性行为，是每个人可以做到的，所以像确立世界艾滋病日的目的，包括有告诉全世界人在世界范围内控制和预防艾滋病可以做到，包括要去理解这些艾滋病患者或者功能携带者，其中还有很重要的一条就是我们每个人要对自己的行为负责。
张羽：我想在今天节目快结束的时候，我还想再次强调，我们应该对艾滋病人群的一个特殊的关爱和宽容。因为可能我们会有一个误区，把这样一个人群固定在他们的区域当中，我们就是安全的，其实他们越隐蔽，越孤立，反而我们是越危险的，因为他们就生活在我们的生活，尤其现在性传播渠道已经成了艾滋病传播的一个主要渠道，这样更容易蔓延到我们普通人。
李小萌：是的，随着现在对艾滋病这个妨碍常识的推广，人们对艾滋病的理解在有很大的变化，但是我们也看到一组比较新的数据，来看看，就是联合国艾滋病规划署跟中国相关机构联合进行一个最新的调查数据，在中国的六个城市，我们来看一下，有47.8的被访者不愿意与艾滋病感染患者一起用餐，有64.9%的人不愿意与艾滋病感染者或者患者一起居住，63.4%不愿意一起接受理发的服务，41.3%不愿意一起工作，30%的被访者认为应该不允许感染艾滋病的学生和健康学生一起在学校里面学习。这里面我觉得看得出，一个是歧视的意识依然存在，还有一个就是对艾滋病常识的普及还是需要更广泛的进行。
张羽：其实刚才小萌已经讲到了艾滋病传播的主要的三种渠道，刚才实际我们所列的所有的前提下，都不会感染到艾滋病。
李小萌：是的，所以我们这个社会对艾艾滋病的这种宽容度，像我们刚才说的，从狭隘的角度讲，如果我们没有这个宽容度，艾滋病不管是携带者还是病人，不公开自己的身份，不去接受检测，不去接受治疗，对于他，对于我们，都是放在一个非常危险的情况下。
张羽：这才是真正的定时炸弹。
李小萌：是的。
张羽：确实刚才小萌刚才讲了，其实迄今为止，从我们最早发现艾滋病，到现在已经有两千多万人被艾滋病夺去了生命，中国现在调查，大概全国有70多万艾滋病患者和感染者，还有30多万人口，他并不知情自己感染了艾滋病，如果我们真正能给他一个宽容关爱的环境，让他们可以自己主动去检测，主动去干预，实际才真正给我们社会带来一种安定。
李小萌：而且能不能及时接受治疗，跟艾滋病患者的这种存活期有密切的关系，如果是在晚期，发展一段时间以后才接受治疗，可能平均寿命是50岁多一些，如果是早期就发现并治疗，生命可以到70多岁，这个概念是完全不一样的，而他能不能及时去检测，及时去接受治疗，确实跟这个社会的宽容度、理解程度有很大的关系。
